Child Care Health Prep: Immunization and Medical Care

What shots does my child need to start child care?

Before or on the child's first day of attendance, the care provider shall have on file a certificate of immunization status form prescribed by the department of health proving the child's full immunization for:

  • Diphtheria;
  • Tetanus;
  • Pertussis (whooping cough);
  • Poliomyelitis;
  • Measles (rubeola);
  • Rubella (German measles);
  • Mumps; and
  • Other diseases prescribed by the department of health

The care provider may accept the child without all required immunizations on a conditional basis if immunizations are:

  • Initiated before or on enrollment; and
  • Completed as rapidly as medically possible.

The care provider may exempt the immunization requirement for the child if the parent or guardian:

  1. Signs a statement expressing a religious, philosophical, or personal objection; or
  2. Provides a physician's statement of a valid medical reason for the exemption. 

Free or Low Cost Health Insurance with Community Health Access Program (CHAP)

CHAP can help anyone. Your children may qualify for free or low-cost insurance. Children up to age 19 can get free medical and dental coverage. Children with single parents, children with two parents, children with working parents, teens living on their own, or teens living with friends. If you already have insurance, Medicaid can fill gaps. You can be employed and your children can still qualify for free or low-cost health insurance.
